Senior Act- (SrAct)

The Senior Assertive Community Treatment Program is a group of social workers trained to serve individuals with severe mental illness. They provide assistance in meeting basic needs such as shelter, clothing and food, help with time management and daily routines, development of basic problem solving skills and effective coping mechanisms, psychiatric monitoring and reliable medication administration and assistance with independent living.

Individuals are eligible for this service if they have had two or more psychiatric hospitalizations within the past three years; a diagnosed mental disorder difficult to manage by outpatient therapy, medication or case management; a history of resistance to traditional treatments and extensive involvement with mental health professionals and/ or judged to have a high probability of psychiatric hospitalizations.

Fees for services are determined on a sliding scale based on income and household size. Some insurance may apply.
