Case Coordination
 
The mission of this program is to enhance the quality of life and sense of independence of older adults through service planning and linkage to needed services. A case coordinator responds to the older adult's unmet needs regarding shelter, nutrition, personal care, home making, safety and security, income and expenses, health care access, home repair and transportation. This includes safety situations that require immediate attention. A home visit is often made to gather information about the person's overall situation. Based on this assessment, the older adult is referred to the appropriate health and human services that are needed. In response to a referral, a case coordinator will make a telephone contact and, as needed, provide an in-home assessment. To be eligible, persons must be at least 60 years old and live in Kalamazoo County. No one is denied service based on inability to make a contribution towards the cost of service. A cost-sharing contribution is determined on a sliding scale based on income.
